You will get your Degree Certificate only if you successfully complete the Degree Course. You have to pass in all Parts of the Examination. Since you have not passed in English, you will have to appear for that Part at the Regular or Supplementary Examination. Since it is almost 16 years since you have completed the course, you may have to write the examination as per the existing scheme. This has to be confirmed from the academic section of the University.

Ministry of Human Resources Development, Department of Higher Education, Government of India has introduced “Central Sector Scheme of Scholarship for College and University Students” to provide financial assistance to meritorious students from low income families for graduate / postgraduate studies in colleges and universities and for professional courses such as Medical, Engineering etc. The total number of scholarships will be divided amongst the State Boards based on the State’s population in the age group of 18-25 years, after segregating share of CBSE and ICSE on the basis of number of students passing out from various Boards in the country. So, when implemented in the state, it should be available for BDS course also. Watch for the announcement of the state.

IIT Madras, Dept of Physics, offers M Tech in Solid State Technology and MSc in Physics for Physics students. You can join MTech in Computer Science there if your qualifying discipline is Physics. The status of your application can be ascertained through two SMS servers. You can send your application number as Text message to 09434709991 or 09434709992. There is also the IVRS facility, the details of which are given in the Prospectus. So make use of these facilities to know whether the application has reached the destination. As per the Prospectus, "No claim of submission of the application will be entertained unless the Acknowledgement Card supports it. No correspondence with regard to theapplication, eligibility, and Admit Card will beentertained unless the application number is quoted. If the Acknowledgement Card does not reach you by January 20, 2009, you must check with the JEEOffice, where your application was sent to". So ensure that the Acknowledgment card is received in time.
You will be able to appear for the examination only if your application reaches the designated address on or before the last date announced for receipr of applications.

The number of Medical Colleges in the Private sector cannot be just listed here since the number is large. The list of colleges recognised by the Medical Council of India, is given at http://www.mciindia.org/apps/search/show_colleges.asp . There is no common fee structure for the MBBS course in these colleges. It varies from state to state and also from Colleg to college. The selection process is also different. Some Colleges have entrance tests at All India level. So the only way out is to closely follow the announcements related to the college of your interest. Information on major entrance examinations are given in this site as and when they are announced.
